Responsibilities:
- Provide skilled nursing care and treatment to geriatric patients in the community
- Implement patient-centered care plans and assist in discharge planning
Requirements:
- HSD or GED required
- Current DE LPN license or compact state license
Job Types: Per diem, Part-time, Full-time
Pay: $22.00 - $28.00 per hour
Expected Hours: 20 – 40 per week
