Job Description
We are looking for experienced Home Care LPNs to join our dedicated team at ChristianaCare. As part of the HomeHealth division, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to developing discharge plans for patients. If you are a compassionate LPN with a current DE license or compact state license, we welcome you to apply for our Full Time, Part Time, or Per Diem positions.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in developing discharge plans for patients.
Requirements
- HSD or GED required.
- Must possess a current DE LPN license or compact state license.
