Responsibilities

• Deliver skilled nursing care and prescribed treatment to geriatric patients.
• Implement patient-centered care plans in line with available resources.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma or GED required.
• Must hold a current DE LPN license or compact state license.
