Job Description
We are looking for an experienced Home Care Licensed Practical Nurse (LPN) to join our ChristianaCare HomeHealth team. As a member of our team, you will provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented, patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
