Job Description
We are looking for a dedicated Home Care LPN to join our ChristianaCare HomeHealth team. As an important member of our organization, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to patient discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement goal-oriented care plans that are patient-centered.
- Participate in developing patient discharge plans.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
