Job Description
We are looking for an experienced Home Health Licensed Practical Nurse (LPN) to join ChristianaCare's dedicated HomeHealth team. You will have the opportunity to work with a state-of-the-art Electronic Health Record system and receive comprehensive OASIS-E training, setting you up for success in your HomeHealth career.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans in coordination with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
Job Details
- Job Types: Full-time, Part-time, Per diem.
- Pay: $22.00 - $28.00 per hour.
- Expected Hours: 20 - 40 per week.
