Job Description
We are looking for a dedicated Home Care Licensed Practical Nurse (LPN) to join our team at ChristianaCare. As a member of our HomeHealth division, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement goal-oriented care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license, or compact state license.
