ChristianaCare HomeHealth is currently seeking dedicated Home Care LPN's to join our team. As an LPN, you will provide skilled nursing care and treatments to geriatric patients in the community, supporting New Castle County, DE. This role involves implementing patient-centered care plans, participating in discharge planning, and ensuring quality care for our patients.
Duties and Responsibilities
- Provide skilled nursing care and prescribed treatments to geriatric patients.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
