Job Description
ChristianaCare HomeHealth is currently seeking experienced Home Care LPNs to join our team in New Castle County, DE. As a valued member of our organization, you will provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning processes.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
