Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients in the community.
- Collaborate on goal-oriented care plans that focus on patient well-being.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
