Duties and Responsibilities
- Provide skilled nursing care to geriatric patients in the community.
- Implement patient-centered care plans in alignment with resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
- Available for full-time, part-time, or per diem positions supporting New Castle County, DE.
- Day or night shift availability, weekends, and holidays as required.
