We are looking for a dedicated Home Health Registered Nurse (RN) to join our team at Interim HealthCare of West Palm Beach. Responsibilities

• Deliver home-based care to patients recovering from illnesses, injuries, and chronic conditions.
• Collaborate with physicians and supervise other healthcare professionals in patient care.
• Conduct patient assessments, monitor progress, and communicate changes to the medical team.
• Educate patients and families on treatment plans, medications, and lifestyle modifications.
• Ensure treatment goals are met and facilitate patient discharge when appropriate.

Requirements

• Minimum 1 year of acute care experience, home healthcare experience preferred.
• Active RN license in FL and CPR certification.
• Familiarity with state and federal home health regulations.
• Strong clinical skills, effective communication, and compassion towards patients.
