INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Case Manager, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. The role focuses on promoting patient wellness, improving care outcomes, efficient health service utilization, and reducing denials of payment. This is a full-time position with a weekend commitment, offering a competitive salary and potential sign-on bonus.

Requirements

• Bachelor's degree in Nursing (BSN) from CCNE accredited schools.
• Professional experience in a specialized field.
• Previous experience as a Utilization Review Nurse, Clinical Care Coordinator, Patient Care Integrator, or RN Case Manager at LifeBridge Health is a plus.
