Job Description
We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Case Manager, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs.
Summary
- Location: Randallstown, MD at Northwest Hospital.
- Full-time position with a weekend commitment involving day and evening shifts.
- Shift Schedule: 4/10-hour shifts weekly, from 8:30 am to 7:00 pm.
- Sign-On Bonus Potential: $15,000.
Responsibilities
- Promote patient wellness and improved care outcomes.
- Ensure efficient utilization of health services.
- Minimize denials of payment through effective care management.
Requirements
- Basic professional knowledge with a Bachelor's degree.
- BSN required from CCNE accredited schools, exemptions available for specific roles.
