Job Description

We are looking for a dedicated Licensed Practical Nurse (LPN/LVN) to join our team at Aveanna Healthcare. As an LPN/LVN, you will provide skilled nursing care following physicians' orders and care plans under the supervision of a Registered Nurse. Your responsibilities will include observing and assessing patient conditions, documenting care activities, educating patients and families, and ensuring infection control measures.

Essential Job Functions

  • Deliver and coordinate quality patient care as per physician orders.
  • Observe and assess patient conditions, reporting changes when necessary.
  • Document all activities, assessments, and care coordination promptly.
  • Implement and update nursing care plans.
  • Take necessary nursing actions and achieve expected outcomes.
  • Recognize and prevent risks in patient care.
  • Comply with Aveanna policies, regulations, and nursing standards.
  • Provide patient education on self-care and prevention strategies.
  • Maintain nursing competency through education and training.

Requirements

  • Graduate of an accredited nursing school.
  • Current and unrestricted state LPN/LVN license.
  • Current CPR certification.
  • Proficiency in clinical assessments, documentation, and compliance.
  • Additional state-specific requirements may apply.

Preferences

  • Six months of recent clinical care experience as an LPN/LVN.
  • Home health experience is a plus.

Physical Requirements

  • Must be able to speak, write, read, and understand English.
  • Ability to lift 50 pounds and reposition patients.

  • Possible exposure to infectious diseases and bodily fluids.
