Job Description
We are looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Home Healthcare team at ChristianaCare. As a crucial member of the team, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or General Education Diploma (GED).
- Current DE LPN license, or compact state license.
