Job Description
We are looking for an experienced Licensed Practical Nurse (LPN) to join our ChristianaCare HomeHealth team. If you are passionate about providing skilled nursing care to geriatric patients in the community and are dedicated to patient-centered care, this role is for you!
Responsibilities
- Give skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented and patient-centered while utilizing available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must hold a current DE LPN license or compact state license.
