Job Description
We are seeking experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN with us, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, implement care plans, and collaborate in developing discharge plans for patients in New Castle County, DE.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented, patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must hold a current DE LPN license or compact state license.
- Experience in geriatrics, home health, pediatrics, or wound care is beneficial.
