Job Description
We are looking for a dedicated Licensed Practical Nurse (LPN) to join ChristianaCare HomeHealth and provide skilled nursing care to geriatric patients in New Castle County, DE. As a part of our team, you will implement patient-centric care plans, participate in discharge planning, and ensure the well-being of our patients.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement goal-oriented care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must possess a current DE LPN license or compact state license.
