Responsibilities
- Provide skilled nursing care and prescribed treatments to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans effectively.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.

$22.00 - $28.00 per hour
