Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our team at ChristianaCare HomeHealth. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. You will work full-time, part-time, or per diem shifts in New Castle County, DE, with opportunities for personal and professional growth.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High school diploma or equivalent required.
- Current DE LPN license or compact state license.
