Licensed Practical Nurse MDS Coordinator

Job Description

We are looking for a dedicated Licensed Practical Nurse MDS Coordinator to join Genesis HealthCare. As part of our team, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure quality patient care and proper reimbursement for services.

Position Highlights

• Lead the interdisciplinary RAI process, ensuring compliance with guidelines and accurate completion of MDS for electronic transmission.
• Contribute to the creation of personalized resident Care Plans, addressing resident needs and expected outcomes.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in healthcare facilities.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficiency in Windows applications; experience with AccuMed is a plus.
• Previous MDS Coordinator experience preferred.
• Current BLS/CPR certification required.
