Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) to join the Aveanna Healthcare team. In this role, you will provide skilled nursing care under the supervision of a Registered Nurse, ensuring the delivery of high-quality patient care according to individual care plans and physicians' orders. Your responsibilities will include continuous patient assessment, care coordination, documentation, patient education, and compliance with regulatory standards.

Essential Job Functions

  • Deliver and coordinate quality patient care in accordance with physician orders.
  • Continuously assess patient condition, report changes, and take appropriate nursing actions.
  • Document all activities, assessments, and care coordination accurately and in a timely manner.
  • Participate in developing and updating nursing care plans.
  • Ensure infection control measures and patient safety in line with OSHA standards.
  • Educate patients and families on disease processes, self-care, and preventive strategies.
  • Maintain nursing competencies through continued education and skill evaluations.

Requirements

  • Graduate of an accredited nursing school.
  • Current, unrestricted state license as a Licensed Nurse.
  • Current CPR certification.
  • Proficiency in clinical assessments and documentation.
  • State-specific requirements based on practice location.

Preferences

  • Six months of recent clinical experience as a Licensed Nurse.
  • Home health experience.

Physical Requirements

  • Ability to perform various physical tasks related to patient care.
