Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. Our organization provides a supportive work environment with cutting-edge technology and comprehensive training in OASIS-E. As an LPN, you will be responsible for delivering skilled nursing care to geriatric patients in the community, implementing care plans, and contributing to discharge planning.
Key Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Contribute to developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.

Compensation
Pay: $22.00 - $28.00 per hour
Expected weekly hours: 20 – 40 per week
