Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. As part of the team, you will provide skilled nursing care to geriatric patients in the community, implementing care plans and contributing to patient discharge planning. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented, patient-centered, and resource-conscious.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.

Pay
$22.00 - $28.00 per hour
