Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our team at ChristianaCare HomeHealth. As an LPN in our Home Care program, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, implementing patient-centered care plans and contributing to discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans.
- Participate in developing patient discharge plans.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
