Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. As an LPN in our Home Health division, you will provide skilled nursing care to geriatric patients in the community, implementing care plans and participating in discharge planning. This role offers Full Time, Part Time, and Per Diem positions with flexible scheduling options and competitive pay rates.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
