- Salary Range: $22.00 - $28.00 per hour based on experience.

Requirements
- High School Diploma or GED required.
- Must hold a current DE LPN license or compact state license.
Responsibilities
- Provide skilled nursing care to geriatric patients in the community.
- Implement patient-centered care plans and participate in discharge planning.
Job Types
Positions available: Full-time, Part-time, and Per Diem.
Weekly Schedule
- Expected Hours: 20 - 40 per week.
- Shifts: Day shift, Night shift, Rotating weekends.
