ChristianaCare HomeHealth is seeking compassionate and dedicated LPNs to join our team. As a Licensed Practical Nurse, you will provide skilled nursing care to geriatric patients in the community. This rewarding role involves implementing care plans, participating in discharge planning, and ensuring the well-being of our patients.
Responsibilities
- Give skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented, patient-centered, and resource-consistent.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must possess a current DE LPN license or compact state license.
