Job Description
The University of Vermont Health Network is seeking a dedicated MDS Coordinator to join our team. In this role, you will work under the general direction of the Director of Nursing Services to ensure the completion of a comprehensive assessment of resident needs and the development of individualized care plans. Collaboration with Nursing Unit Managers is key to success, fostering effective communication and teamwork in providing high-quality care.
Responsibilities
- Perform comprehensive resident assessments.
- Develop personalized care plans to meet resident needs.
- Communicate resident status and changes with the health care team.
- Adhere to Vermont Nurse Practice Act, Code of Ethics for Nurses, and Nursing Department policies.
- Support the philosophy and goals of CVMC/Woodridge and the Nursing Department.
Requirements
- Graduation from an accredited Nursing Education Program with a current RN licensure.
- 5 years of MDS experience post-2019.
- Three to five years of experience in long-term care as a Team Leader or Charge Nurse.
