Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is seeking a passionate and detail-oriented Minimal Data Set Coordinator who is a Licensed Practical Nurse (LPN). As an integral part of our healthcare team, you will participate in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS), ensuring optimal patient care and accurate nursing center reimbursement.

Responsibilities

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process in compliance with regulations.
• Develop individualized resident Care Plans based on resident needs and regulatory requirements.
• Train team members on clinical content and electronic MDS completion.

Requirements

• LPN in good standing with current state license.
• Prior experience in a healthcare facility.
• Familiarity with nursing practices and regulations.
• Proficiency in windows applications; AccuMed experience preferred.
• Previous experience as an MDS Coordinator is a plus.
