Minimal Data Set Coordinator - LPN

Job Description

We are looking for a dedicated Minimal Data Set (MDS) Coordinator - LPN to join Genesis Health, a leading healthcare service provider. As part of our team, you will be responsible for participating in the Resident Assessment Process by completing relevant items on the MDS and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Direct the RAI process and ensure accurate completion, encoding, and transmission of the MDS.
• Contribute to the development of individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• Current LPN license in good standing.
• Prior experience in a hospital, long-term care facility, or similar healthcare setting.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Familiarity with laws, regulations, and guidelines related to long-term care.
• Computer skills, including proficiency in Windows applications.
• AccuMed experience preferred.
• Prior experience as an MDS Coordinator preferred.
• Strong understanding of RAI/PPS processes.
• Current BLS/CPR certificate (excluding ALFs/ILFs).
