Job Description
Genesis Healthcare is seeking a Nurse-LPN MDS Coordinator to join our team. As a vital part of the Resident Assessment Process, you will complete the Minimum Data Set (MDS) and Care Area Assessments (CAAs), ensuring proper patient care and nursing center reimbursement.
Position Highlights
- Lead the interdisciplinary RAI process, following guidelines for MDS completion and electronic transmission.
- Create individualized resident Care Plans in compliance with Federal and State regulations.
- Provide training on clinical content and electronic MDS completion to new team members.
Requirements
- LPN in good standing with current state license.
- Prior experience in hospital or long-term care facilities.
- Familiarity with general, rehabilitative, and restorative nursing practices and regulations.
- Proficient in computer applications, preferably with AccuMed experience.
- Experience with MDS completion and Care Plans.
- Strong knowledge of RAI/PPS processes.
