In this role, you will provide skilled nursing care to patients under the supervision of a Registered Nurse, following care plans and physicians' orders while maintaining confidentiality and professionalism. Responsibilities

• Deliver and coordinate quality patient care in adherence to physician orders.
• Observe and assess patient condition, report changes, and document care activities promptly.
• Participate in care planning, take necessary nursing actions, and prevent/respond to changes in patient needs.
• Adhere to clinical assessments, documentation, and nursing care compliance.
• Educate patients and families on disease processes, self-care, and prevention strategies.

Requirements

• Graduate of an accredited nursing school.
• Current, unrestricted state license as a Licensed Nurse.
• Current CPR certification.
• Proficiency in clinical assessments, documentation, and nursing care compliance.
• Additional state-specific requirements may apply.

Preferences

• Recent experience as a Licensed Nurse in a clinical care setting.
• Home health experience.

Physical Requirements

• Must be able to speak, write, read and understand English.
