Responsibilities

• Conduct home visit Physical Therapy assessments to evaluate patients' conditions.
• Provide direct patient care and create personalized treatment plans.
• Document patient progress accurately in our EMR system.
• Collaborate with patients, families, and healthcare providers to achieve optimal outcomes.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Proficient in EMR documentation.
• CPR Certification.
