• Graduated from an accredited Physical Therapy college or university.
• Hold an unencumbered Physical Therapy license for New York.
• Conduct home visit assessments to provide direct patient care and assess medical conditions, functional capabilities, and potential for rehabilitation.
• Proficient in EMR documentation.
• CPR Certification.
