• Conduct home visit assessments to evaluate patients' medical conditions and rehabilitation potential.

• Proficiently document patient information using electronic medical records.

Requirements:

• Graduate of an accredited Physical Therapy program.
• Valid Physical Therapy license for New York.
• CPR Certification.
• Experience with EMR documentation preferred.
