Physical Therapist Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Valid and unencumbered Physical Therapy (PT) license in New York.
• Ability to conduct home visit assessments and provide direct patient care.
• Proficiency in EMR documentation.
• CPR Certification.
