We are looking for a dedicated Physical Therapy Specialist to join our team at Paradigm Rehab. Our ideal candidate is passionate about helping individuals enhance their quality of life through physical therapy. As a Physical Therapist with us, you will conduct home visits, assess patients' medical conditions, and develop rehabilitation plans to improve their functional capabilities.

Responsibilities

• Conduct home visit assessments and provide direct patient care.
• Document patient progress accurately in an EMR system.
• Collaborate with patients to develop customized rehabilitation plans.
• Ensure CPR certification and compliance with licensing requirements.

Requirements

• Graduate from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Proficient in conducting assessments and developing treatment plans.
• Experience with EMR documentation is an advantage.
