Paradigm Rehab is looking for a dedicated Physical Therapy Specialist to join our team. As a Physical Therapist, you will play a key role in improving the quality of life for our patients by providing direct care and facilitating the rehabilitation process.

Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions and functional capabilities.
• Provide direct patient care and develop tailored treatment plans for rehabilitation.
• Utilize EMR documentation for accurate record-keeping.
• Collaborate with a compassionate team to ensure the best care for patients.

Requirements

• Graduated from an accredited Physical Therapy program.
• Valid Physical Therapy license in New York.
• Proficiency in EMR documentation.
• CPR Certification.
