Physical Therapy Specialist Responsibilities:

• Conduct home visit assessments to evaluate patients' conditions and potential for rehabilitation.
• Provide direct patient care and support to enhance functional capabilities.
• Utilize EMR documentation efficiently.

Physical Therapy Specialist Requirements:

• Graduate from an accredited Physical Therapy program.
• Hold a Physical Therapy license in New York.
• Possess CPR certification.
• Experience with EMR (Hello Note experience preferred).
