Responsibilities
- Conduct home visit assessments to evaluate patients' medical conditions and rehabilitation potential.
- Provide direct patient care and develop personalized treatment plans.
- Document patient records accurately using EMR systems.
- Ensure patients receive the highest quality of care and attention.
Requirements
- Graduated from an accredited Physical Therapy program.
- Hold an active Physical Therapy license in New York.
- Proficient in EMR documentation.
- Current CPR Certification.
