Primary Care Physician (Bilingual Spanish)

Job Description

Esperanza Health Centers is seeking a compassionate and skilled Primary Care Physician (Bilingual Spanish) to join our team. As a Primary Care Physician, you will provide essential medical services to older adults participating in our Program of All-Inclusive Care for the Elderly (PACE), focusing on preventive care and promoting healthy lifestyles. In addition to direct patient care, you will play a key role in leading and training other healthcare professionals while collaborating within our interdisciplinary team.

Primary Duties and Responsibilities

• Manage primary care services in the PACE Center and other post-acute settings.
• Participate in IDT meetings, care coordination, and quality improvement initiatives.
• Collaborate with team members to create individualized care plans for PACE participants.
• Ensure quality care delivery within the PACE model.
• Coordinate with partner facilities and providers for comprehensive participant care.

Requirements

• Bilingual (English and Spanish) fluency is mandatory.
• Board certification in primary care with a preference for geriatrics specialization.
• State of Illinois physician license.
• Valid driver's license and current automobile insurance.
• Compliance with vaccination requirements of partner facilities.
