Essential Job Functions

• Deliver and coordinate quality patient care in line with physician orders.
• Observe and assess patient condition, report changes, and update care plans accordingly.
• Document all care activities, assessments, and nursing actions promptly.
• Participate in the development and execution of nursing care plans.
• Take necessary nursing actions based on assessment findings.
• Recognize and address changes in patient needs promptly to prevent risks.
• Comply with Aveanna policies, regulations, and infection control measures.
• Educate patients and families on self-care and disease management.

Requirements

• Graduate of an accredited nursing school.
• Current state license as an LPN.
• Valid CPR certification.
• Proficiency in clinical assessments and documentation.

Preferences

• 6 months of recent clinical nursing experience.
• Experience in home health care.

• Attention to detail, time management, and problem-solving skills.
• Strong organizational and communication abilities.

Physical Requirements

• Must be able to lift up to 50 pounds and assist patients with mobility.
• Ability to move and position patients and equipment independently.
