Job Description

We are looking for a dedicated Registered Nurse (RN) Field Nurse for Home Care to join our team at ChristianaCare. In this role, you will provide skilled nursing care to geriatric patients in the community, develop and implement care plans, and ensure a patient-centered approach. Our supportive leadership and state-of-the-art Electronic Health Record system make this a great opportunity for experienced Home Health nurses.

Responsibilities

• Provide skilled nursing care and prescribed treatment to geriatric patients.
• Develop and implement patient-centered care plans.
• Prepare and execute discharge plans for patients.

Requirements

• BSN required; ADN candidates must obtain BSN within three years of hire.
• Current DE RN license or compact state license.
• 2 years of recent Home Health RN experience.
• Previous experience with Home Healthcare and Med-Surgical preferred.
• IV proficiency or willingness to train for IV functions.
