Job Description
We are looking for a dedicated Registered Nurse (RN) Field Nurse specializing in Home Care to join our team at ChristianaCare. You will have the opportunity to provide skilled nursing care and treatment to geriatric patients in the community, with a particular emphasis on developing patient-centered care plans and ensuring efficient discharge processes.
Responsibilities
- Provide skilled nursing care to geriatric patients in the community.
- Develop and implement patient-centered care plans.
- Facilitate discharge planning for patients.
- Must be IV proficient or willing to train and perform IV functions.
Requirements
- BSN required; ADN candidates must obtain their BSN within three years.
- Current DE RN license or compact state license.
- 2 years of recent Home Health RN experience.
- Previous Home Healthcare and Med-Surgical experience preferred.
