Job Description
We are looking for a Full-Time Registered Nurse (RN) Field Nurse for Home Care at ChristianaCare. The successful candidate will provide skilled nursing care and treatments to geriatric patients in the community, ensuring patient-centered care plans and successful patient discharges. Responsibilities
- Administer skilled nursing care and treatments to geriatric patients in the community.
- Develop patient-centered care plans and ensure goal-oriented care.
- Prepare patients for discharge and support transitions.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of employment.
- Current DE RN license or compact state license.
- Minimum of 2 years of recent HomeHealth RN experience.
- IV proficiency required or willingness to train.
