Job Description
We are currently looking for a passionate Registered Nurse (RN) specializing in Home Health to join our team at Atrium Health. This full-time position entails providing exceptional healthcare services to adult and geriatric patients in the comfort of their homes. As our dedicated Registered Nurse, you will play a crucial role in coordinating care, ensuring patient needs are met, and maintaining high-quality standards.
Job Responsibilities
- Serve as a case manager for assigned patients, overseeing post-acute services in home settings.
- Provide direct patient care and supervise LPNs and HHAs in accordance with care plans.
- Coordinate care with a multidisciplinary team and act as a liaison with other healthcare providers.
- Ensure timely care, proper documentation, and high-quality outcomes for patients.
Requirements
- Graduate from an accredited School of Nursing, BSN preferred.
- Current RN license and BLS certification required.
- Experience in home health care is advantageous.
- Ability to prioritize and manage patient care effectively.
