Job Description
We are looking for a compassionate and dedicated Registered Nurse to join our Post Acute Unit at UVMHN Helen Porter Rehabilitation & Nursing. Our unit provides goal-directed care for patients post-surgery or hospitalization, with a focus on enhancing independence and promoting recovery.
Responsibilities
- Administer quality nursing care to patients in accordance with individual care plans.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Provide education to patients and families to support transition back home.
- Maintain accurate patient records and monitor vital signs.
Requirements
- Valid RN license in the state of Vermont.
- Previous experience in a skilled nursing facility or acute care setting preferred.
- Strong communication and organizational skills.
- Ability to work independently and as part of a team.
