Rehabilitation Therapist - $70-$90 per Hour

Are you passionate about helping individuals improve their quality of life through rehabilitation therapy? Paradigm Rehab is seeking a dedicated Rehabilitation Therapist to join our team. As a key member of our organization, you will work closely with patients in their homes to assess their medical conditions, functional capabilities, and potential for rehabilitation.

Responsibilities

• Conduct home visit assessments of referred patients.
• Provide direct patient care to assess and improve functional abilities.
• Document patient information in our EMR system.
• Collaborate with interdisciplinary teams for comprehensive patient care.

Requirements

• Graduated from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Proficient in EMR documentation; Hello Note experience is a plus.
• CPR Certification.
